Report concerns about student conduct, safety, academic misconduct, or other incidents that require staff attention. All reports are reviewed by the Office of Student Affairs.
In an immediate emergency, call 911. This form is not monitored 24/7 and is not a substitute for emergency services. For campus security, call 205.929.6350.
What to Report
Academic Misconduct
Plagiarism, cheating, fabrication of data, academic dishonesty by another student.
Behavioral / Conduct Violation
Disruptive, threatening, or inappropriate behavior by a student in a classroom, on campus, or in college-related activities.
Distressing / At-Risk Behavior
A student who appears to be in crisis, exhibiting signs of mental health distress, or at risk of harming themselves or others.
Harassment or Discrimination
Harassment based on race, gender, religion, national origin, disability, or other protected characteristics.
Title IX / Sexual Misconduct
Sexual harassment, assault, dating violence, stalking, or other behaviors covered under Title IX.
Property Damage or Theft
Damage to college property or theft of equipment, personal property, or college materials.
How It Works
Ensure Immediate Safety
If anyone is in immediate danger, call 911 first. For campus emergencies, call Campus Security at 205.929.6350. Do not submit this form as a substitute for emergency help.
Gather What You Know
Note the date, time, location, and persons involved. If there are witnesses, note their names. You don't need to have every detail — submit what you have.
Complete the Report
Fill out the Student Incident Report form with as much detail as possible. Reports can be submitted anonymously, but providing your contact information allows staff to follow up with you.
A Staff Member Follows Up
The Office of Student Affairs reviews all submissions and will follow up with the reporting party if contact information was provided. Response time is typically 1–3 business days.
